Our staff will open your file and enter the necessary information (insurance policy number, personal data, medical and dental history, etc.). We will also be able to answer your questions about our services and fees and schedule your next visits with you.
Medical Form
In order to provide you with the best possible care, we need to know about your general and oral health. To do so, we ask that you complete the following medical questionnaire before your first visit. Simply download it, fill it out and send it to us or print it and bring it to your appointment. Be sure to include an up-to-date list of your medications. The data contained in the form will help our dentist plan your dental care and adapt his or her actions as needed. It will be kept secure and confidential and only the appropriate people will have access to it if needed.

Insurance
We accept all dental insurance. If your plan allows it, we will forward your claim directly. We also accept payment delegation. Otherwise, we will provide you with the necessary information that you will have to forward yourself. We can also transfer an estimate of upcoming treatments so that you know in advance what is covered and what is not. Please note that the remaining balance will be charged to you and you will have to pay it in full. The amounts reimbursed vary between insurance policies, which is why it is important to be well informed on this subject.
If you wish, our reception staff can help you understand your policy during your first appointment. Please bring the related documents with you, since we do not necessarily have this information. We recommend that you contact your insurance company directly if you have any questions or misunderstandings. This way, you will be better able to plan your care according to your real needs and means.
